What to expect on your first visit?
Walk into our practice, where you will be greeted by our friendly team. We will provide you with new patient paperwork that includes a health questionnaire, request for your insurance information and contact information. We will also provide you with information to familiarize you with our office and how we handle things like patient privacy. You can also download these forms from our website so that you can read and complete them before your visit and bring along with you.

During your first visit, we will review your dental and medical history forms, and take X-rays of your teeth and mouth. Then, Dr. Portillo will meet with you, review your X-rays, listen to your goals and concerns and create your individual treatment plan. At the next visit, our hygienist will clean and polish your teeth, checking your teeth and gums for any cavities or early signs of tooth decay or gingivitis.
